Case manager is one title for several different jobs. In a nonprofit or county agency, you help people get housing, benefits, treatment or safety. In a hospital or health plan, you plan discharges and coordinate care, often as a nurse or social worker. How to write a case manager resume

Name your setting and population

Under your name, a headline such as Case Manager, Housing and Homeless Services, RN Case Manager, Utilization Review or Case Manager, Workers' Compensation. Then say in the summary who you serve: families, older adults, people leaving incarceration, youth in foster care, adults with serious mental illness. Supervisors hire for experience with their population.

Lead with your caseload

The first bullet for each job gives the number of clients or cases you carry and how you work them: home visits, office appointments, phone check-ins, hospital rounds. A caseload of 15 high-need clients and one of 80 phone-based cases are very different jobs, so be precise.

Show outcomes, not only activity

Every program reports outcomes to a funder or payer, so you likely know yours: people housed and still housed a year later, benefits approved, readmissions avoided, days to discharge, return-to-work rates, goals met on service plans. Pick the two or three that matter most to the job you want.

Include the paperwork side of the job

Case management depends on good records. Name the system you document in (HMIS, Epic, a state child welfare system, a claims platform), how quickly you enter notes, and any audits or reviews you passed. Supervisors worry about documentation more than applicants expect.

A social work license, a registered nurse license, the CCM, or a state certification for your program type goes near the top if the job requires it, and after education if it is a plus. Add training such as Mental Health First Aid, motivational interviewing or trauma-informed care, with the year.

Licenses and certifications for case managers

What you need depends on the setting. Many social services case manager jobs ask for a bachelor's degree and experience rather than a license. Hospital, health plan and clinical roles often require a nursing or social work license. Licensing rules are set by each state, so check your state board before you apply. These are the credentials employers mention most:

  • Social work license: issued by your state's social work board. The exams are developed by the Association of Social Work Boards (ASWB) at five levels (Associate, Bachelors, Masters, Advanced Generalist and Clinical), and each state decides which licenses and exams it uses.
  • Registered nurse license: required for nurse case managers. You earn it by finishing an approved nursing program and passing the NCLEX-RN exam, then applying to your state board of nursing.
  • Board-Certified Case Manager (CCM) from The Commission, known until 2026 as the Commission for Case Manager Certification (CCMC). As of September 2026, you qualify with a license such as RN or LCSW, or a bachelor's or master's degree in a health or human services field, plus either 12 or 24 months of full-time case management experience or 12 months of full-time experience supervising case managers, depending on your route.

Mistakes that weaken case manager resumes

  • No caseload size. The number of clients you carry tells a supervisor more about your experience than any description. Put it in the first bullet.
  • Only tasks, no outcomes. Provided referrals and completed assessments describes every case manager. Add what happened: housed, approved, discharged safely, back at work.
  • Anything that identifies a client. Names, specific stories and details that could point to a person break confidentiality and worry employers. Keep examples general.
  • Not naming the population. Older adults, families, veterans and people with disabilities each need different knowledge. Say who you have served.
  • A license listed without the state. Social work and nursing licenses are issued by states. Write the license type and the state, and whether it is active.
  • Skipping documentation. Programs depend on records for funding and audits. Name your system and show you keep notes current.
